SAMPLE. D-02, first-floor bedroom door: FAIL. Gap at the head measured 6 mm and on the latch side 5 mm, outside the usual 2 mm to 4 mm range. Smoke seal missing along the latch edge and the closer does not pull the door into the latch.
Adjust or rehang the leaf to bring the gaps within range, fit new fire and smoke seals and adjust or replace the closer. Re-inspected after repairs: PASS, report updated.
How failed doors are written up
A failed door lists each fault with the measurement or photo behind it. The repairs needed sit underneath, so a fitter can price the work straight from the report.

FAQ
Questions about using the report.
Is the report accepted by the council?
Each council sets its own requirements, so check with your licensing officer what they need. The report gives a pass or fail for each door with photos and the repairs needed, so you have door-by-door evidence to show them.
Can I share it with my fire risk assessor?
Yes. The report is yours to share with your fire risk assessor, insurer or managing agent.
Do you give a certificate?
I give you an inspection report, which records a pass or fail for every door on the day of the visit. There's no general legal rule that every landlord needs an annual fire door certificate.
